Job Description

GENERAL DESCRIPTION

Under the direction of the Community Health Representative Manager, the Public Health Nurse provides community-based nursing services through home visits, patient assessments, development of care plans, and delivery of treatment. The PHN offers health education, advocacy, and referrals to appropriate programs to address medical, social, and emotional needs. The role maintains ongoing communication with leadership and the community regarding health trends and concerns, and is responsible for assessing, planning, implementing, and evaluating the comprehensive Public Health Nursing Program for Ohkay Owingeh.

NATURE OF WORK

ESSENTIAL FUNCTIONS

  • Monitor and assess community health status, trends, and risk factors.
  • Conduct community assessments, epidemiological surveys, field investigations, and data analysis to identify health needs.
  • Investigate communicable diseases and recommend appropriate interventions.
  • Collaborate with tribal, local, state, and federal partners in planning and implementing public health activities.
  • Partner with community members and stakeholders to identify priorities and design targeted interventions.
  • Develop and implement policies, plans, and actions that support individual, family, and community health.
  • Mobilize resources and coordinate efforts to address identified health concerns.
  • Provide health education and coaching to individuals, families, and groups in various settings (schools, jails, clinics, homes).
  • Promote informed decision-making regarding health issues and ensure adherence to epidemiological practices.
  • Support community capacity-building and contribute to sustainable health-promoting programs.
  • Conduct patient assessments and home, school, clinic, and community visits.
  • Develop and implement care plans; provide preventive, curative, and rehabilitative nursing services.
  • Administer treatments as prescribed and participate in case management with healthcare providers.
  • Educate patients and families on diagnoses, treatment plans, and health responsibilities.
  • Reevaluate patient needs and adjust care plans accordingly.
  • Participate in program evaluation, quality improvement, and development of standards of care.
  • Maintain accurate documentation, reports, and participant files.
  • Assist with grant planning, applications, compliance, and reporting requirements.
  • Support program promotion, community outreach, and event planning.
  • Communicate regularly with grantors and participate in trainings, site visits, and leadership meetings.
  • Maintain cultural sensitivity and work effectively with diverse populations.
  • Protect confidentiality and uphold tribal policies and procedures.
  • Demonstrate strong clerical, computer, and data management skills.
  • Maintain professional knowledge through training, workshops, and continuing education.
  • Contribute to a team environment and support agency mission and goals.
  • Travel within the community and occasionally out of state as required.
  • Performs other duties as required.

 

 

  • KNOWLEDGE, ABILITIES, SKILLS, AND CERTIFICATIONS:
  • Knowledge of traditional form of government and pueblo customs and traditions.
  • Knowledge of the functions and structure of Ohkay Owingeh.
  • Knowledge of public health principles, epidemiology, and population-based nursing.
  • Knowledge of chronic and communicable disease management, prevention, and intervention strategies.
  • Knowledge of community health assessment methods and data collection techniques.
  • Ability to work independently and exercise sound clinical judgment.
  • Ability to build and maintain effective relationships with patients, families, and community partners.
  • Ability to prioritize multiple tasks, meet deadlines, and manage unpredictable situations.
  • Ability to maintain patient confidentiality and handle sensitive information appropriately.
  • Strong clinical assessment and diagnostic skills.
  • Skill in developing, implementing, and evaluating patient care plans.
  • Skill in conducting health education, outreach, and community engagement.
  • Skill in maintaining accurate records, documentation, and reporting.
    • Skill in using electronic health records (EHR), databases, Microsoft Office Suite, and health data systems.
  • Skill in analyzing data, identifying trends, and making evidence-based recommendations.
  • Public Health Nurse (PHN) Certification – preferred
  • Basic Life Support (BLS) – preferred
  • Emergency Preparedness & Response Training – preferred ability to supervise and ensure a safe learning environment for young children.
  • Ability to demonstrate moral character, honesty, tact, fairness, lack of prejudice, and desire to help when dealing with people.
  • Certified in First Aid and CPR within 90 days of employment.
  • Oneida certification on reporting Child Abuse and Neglect within 90 days of employment.

 

 

PHYSICAL DEMANDS:

While performing the duties of this job, the employee regularly is required to stand; sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to walk. The employee occasionally is required to stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Maintain physical stamina to conduct home visits, community outreach, and field activities throughout the Pueblo. Maintain mental resilience while working with individuals experiencing medical, behavioral, or emotional distress.

 

 

MINIMUM QUALIFICATIONS, EDUCATION, & EXPERIENCE

  • Current valid New Mexico Driver’s License.
  • Must meet Pueblo insurance eligibility requirements.
  • Must successfully pass a pre-employment drug/alcohol screen, and background investigation.
  • Bachelor’s Degree in Nursing (BSN) from an accredited nursing program.
  • five (5) years of professional nursing experience in a public health or tribal community setting; or an equivalent combination of education and experience.

 

 

WORK ENVIRONMENT:

The work environment characteristics described here are representative of those encountered while performing the essential duties of this position. Work is performed in a variety of settings, including office, clinic, homes, schools, and community locations throughout the Pueblo. Noise levels vary from quiet (office) to moderate or high during community events, clinics, or home visits. Regular travel within the community is required; occasional travel out of the area may be needed for trainings, meetings, or conferences.

 

The position may involve exposure to varying weather conditions during home or community visits. Possible exposure to communicable diseases, bloodborne pathogens, and environmental hazards; appropriate personal protective equipment (PPE) is provided and required. Work schedules may include evenings, weekends, or holidays based on public health needs, events, or emergencies. The role may require on-call availability for urgent public health matters or emerging health concerns.
