The Ohkay Public Health Nurse began services on July 30,2018. The tribe elected to take their 638 shares for this position so that services would be located in the community.
Services provided by the PHN are population based with an emphasis on Primary Prevention. This includes promoting health, protecting against health threats to the community, and keeping problems from occurring in the first place. These services are provided to individuals, families, community and systems.
PHN services strive to be comprehensive and culturally appropriate for personal and public health.
Describes and monitors health events for purpose of planning, implementing, and evaluating public health interventions
Gathers and analyzes - data regarding threats to the health of populations, ascertains the source of threat, identifies cases and others at risk, and determines control measures.
Locates populations of interest or at risk and provides information about the nature of the concern; what can be done and how services can be obtained. Provide health related education topics to schools, elder groups, parents, and others as identified.
Locate individuals and families with identified risk factors and connects them to resources
Identifies individuals with unrecognized health risk factors or asymptomatic disease conditions. Coordinate and provide school health services for Headstart and Elementary Schools.
Assist with individuals, families, groups, organizations, and communities to utilize necessary resources to prevent or resolve problems or concerns.
Assists individuals or families by assessing, planning, implementing, coordinating services needed, evaluating if those services were helpful; for health needs
